[Flex] NEW Fastlane COI form

While the instructions are "in the way," my inclination is to leave the 
form alone and return it, completed, just as it is.

I don't really know, but I think NSF probably probably runs all the 
spreadsheets through a routine that  "sorts" the various tables so 
different review groups or program managers get the info they need.

Since it's NSF's form, I say we just complete it and return it.

That's my thinking, anyway,
