[Flex] Draft FLEX meeting notes

All --I was unable to attend this week's Flex meeting, but I have issues 
with the NOAA 424 and the 424a forms.  There seems to be disagreement 
among folks about exactly what information is included on these forms.  
I think we're all completing them a bit differently.  I've looked over 
the instructions again (and again ) and have created an example of what 
I think is the correct way to complete them, but maybe I'm all wrong !!  
How can we most conveniently discuss this ?  Thanks.   MJ
